I agree that Frequency & Flow Wellness & Megan Coats, shall not be liable for any claim, demand or cause of action of any kind or account of personal injury, property damage or loss of any kind resulting from or related to activities or services provided.
I understand that if I need accommodations due to health, physical or mental health concerns, I will address these prior to the activities or services provided.
Thie Waiver of Liability will hold valid and true for the present and future, regardless of circumstances and changes in the law.
